The Ozempic Pill: Five Semaglutides, Three Shared Doses

There is now an Ozempic pill, and it is not Rybelsus, and it is not the Wegovy tablet, and all three are semaglutide made by the same company. Three of the tablet strengths appear on two different products with different indications, and the label describes those tablets in identical words. A white round tablet debossed with a 4 is not enough information to know what someone is taking.

The five products, and what each number means#

Everything below is read from the RYBELSUS and OZEMPIC tablets prescribing information and the WEGOVY prescribing information.

Research suggests the reason the numbers look chaotic is that they are answering three different questions at once: how much survives digestion, what the target condition is, and which formulation generation the product belongs to.

The collision worth knowing about#

Ozempic tablets and Wegovy tablets share 1.5 mg, 4 mg and 9 mg.

The label describes both products' tablets the same way: white to light yellow, round, debossed with the strength on one side and "novo" on the other. So a 4 mg tablet is either:

• an Ozempic maintenance dose for type 2 diabetes, where 4 mg may be where treatment stays, or
• step two of four on a Wegovy weight ladder heading to 25 mg.

Same molecule, same manufacturer, same tablet face, different medicine. The distinguishing information is on the box and the prescription, not on the tablet.

Ozempic pill replaces Rybelsus, and the swap is not one-for-one#

Studies have shown oral semaglutide works only with an absorption enhancer alongside it, and Rybelsus was the first product to use one. Ozempic tablets are the same molecule in an updated formulation with higher bioavailability, which is why the milligram figures are lower for a comparable effect.

Two consequences the label spells out.

Existing Rybelsus patients need a new prescription. It is not a relabelling that carries over.

The two are not substitutable on a mg-to-mg basis. That phrase is the label's, not a paraphrase. The mapping it gives instead:

So 7 does not become 7, and someone reading only the milligram figure would conclude their dose had been halved. It has not. The formulation changed underneath it.

The label also says not to switch between them during the initiation phase, only after the first 30 days.

The starting dose that is not meant to work#

Both diabetes tablets open with a step the label describes as not effective for glycemic control: 3 mg for Rybelsus, 1.5 mg for Ozempic tablets, each for 30 days.

Research has shown the escalation exists to blunt gastrointestinal effects rather than to build up an effect, and that is unusual enough to state clearly. It is a tolerability run-in, not an underdose, and a reader who concludes the drug has failed after a month on the first step has misread what the first step is for. The same pattern appears on the Wegovy tablet ladder, where 1.5 mg is an initiation dose on the way to 25.

Switching from the injection#

The label covers this too, and the numbers are again not intuitive.

One week after stopping 0.5 mg Ozempic injection, a patient may start 4 mg or 9 mg Ozempic tablets, or 7 mg or 14 mg Rybelsus. For the weight products, one week after stopping 2.4 mg Wegovy injection, a patient may start 25 mg Wegovy tablets.

The short version#
